Connect Up 4 Kids Healthy Weight Pathway

The Connect Up 4 Kids Healthy Weight Pathway provides a guide for General Practitioners, Practice Nurses and Allied Health Professionals in assessing children aged between 3 and 7 years around healthy weight and provides a visual guide to the available referral options in the ACT for those children who are outside the healthy weight range.

The pathway encourages practitioners to assess all children between the ages of 3 and 7 opportunistically, "Every Child, Every Visit".

The pathway also incorporates the child health assessment points of the Healthy Kids Check and the ACT Kindergarten Health Screen, where assessment of the height and weight of the child is likely to be undertaken as part of the examination. encourages practitioners to assess all children between the ages of 3 and 7 years.
