The members will present with a Defence Approval Number (DAN).  To bill, send an invoice that includes the member’s full name (include rank), date of birth, address, DAN number and PMKeys number (seven digit number starting with 8, may also be written as EPID number on a referral) to BUPA Customer Service Office via
