Heart Failure Care Initiative – Clinical Leadership Forum

Capital Health Network is seeking expressions of interest for GP, Community Pharmacist, Practice Nurse and allied health positions on the Heart Failure Care Clinical Leadership Forum.

CHN, through the ACT PHN programme, is developing an innovative approach to the development of a comprehensive person-centred and whole-of-system approach to heart failure care.  This initiative will be clinician led and delivered in collaboration with key stakeholders including primary health care professionals and local specialists, and strategic partners encompassing ACT Health, Health Care Consumers’ Association and the Heart Foundation.

To drive this initiative we are establishing a Clinical Leadership Forum to shape the development and champion the implementation of this system-wide innovation.

Expression of Interest – GPs and Practice Nurses: Transition of Care Pilot (ToC) – Project Steering Group

Capital Health Network, through the ACT PHN programme, is currently seeking expressions of interest for both the GP and Practice Nurse positions on the ToC Project Steering Group.

The transition of care between hospital and home is a key point in health care delivery where adverse events and disruptions to continuous and comprehensive care often occur. Poor transitions of care impact on patient safety, the quality and experience of care and the overall performance of the ACT’s health system.

CHN has developed and plans to pilot a Transitions of Care Service Delivery Model which adopts an evidence-based system wide approach to ‘improve patient focused transitions of care between hospital and primary health care and community settings’.

This model will be piloted to June 2018 on a collaborative basis with Division of Medicine, The Canberra Hospital. Further information is available here.
