Heart failure is a chronic, complex, non-curable condition affecting an estimated 300,000 Australians, with 30,000 new cases diagnosed each year. Factors like polypharmacy, diagnosis (missed, mistaken, late), repeated exacerbations of symptoms, lack of self-care strategies and care integration between hospital and primary care providers further complicate this life-threatening condition.

Our team coordinates a quality improvement project with a focus on heart failure. The project aims to improve outcomes for patients with or at risk of heart failure through encouraging increased engagement with their general practice. This project involves the implementation of multiple strategies to improve heart failure management and preventative care including strategies for improvements in data accuracy and data cleansing, increased cardiovascular risk assessment, improved completion of CDM item numbers together with increased support for patients with heart failure.

Practices need to identify a team member to lead the quality improvement activity and commit to undertake at least four of the above strategies
