ITC Patient Referral, Intake and Management process

ITC GP Forms
a) Practice Registration
Complete the Practice Incentives Program Indigenous Health Incentive practice register authority form (IP009) for your practice details to be included on the Practice Incentives Program (PIP) Indigenous Health Incentive practice register.
b) Patient consent and patient registration forms
To demonstrate compliance with privacy legislation, practices must obtain and record each patient’s informed consent for the use and disclosure of personal information by completing the patient consent form. In addition to providing a verbal explanation of the Indigenous Health Incentive and/or the PBS Co-payment Measure, practices should provide patients with a copy of the patient information sheet, which is also available on the Medicare website. The GP should be satisfied that the patient has a sufficient understanding of the incentive prior to asking them if they wish to be registered for the Indigenous Health Incentive and/or the PBS Co-payment Measure. Both the patient consent and patient registration forms should be kept on file at the practice (either in hard-copy or electronically). A copy of the patient registration form should be sent to Medicare Australia to claim a patient registration payment.
c) Practice Incentives Program Indigenous Health Incentive and Pharmaceutical Benefits Scheme Co-Payment Measure Patient Registration and Consent form (IP017)
The Government has introduced assistance to eliminate or lower the cost of Pharmaceutical Benefits Scheme (PBS) medicines for Aboriginal patients living with, or at risk of, chronic disease. The PBS Co-payment Measure is a response to identifying that cost can be a significant barrier to accessing medicines for Aboriginal people.
Use this form for patient registration if you are a practice or Indigenous health service participating in the Practice Incentives Program (PIP) Indigenous Health Incentive and would like to register eligible Aboriginal or Torres Strait Islander patients for the Indigenous Health Incentive and/or the Pharmaceutical Benefits Scheme (PBS) Co-payment Measure.

d) Referring eligible patients to the CHN Close the Gap program for Care Coordination or Supplementary Services support
Use this form for referral of eligible patients (under the PIP Indigenous Health Incentive) to the CHN Close the Gap Program for enrolment to receive Care Coordination and/or Supplementary Services support.
e) Referring to Allied Health Services under Medicare for People of Aboriginal or Torres Strait Islander Descent
Use this form for referring eligible patients to Allied Health services identified as part of the GP Management Plan or Team Care Arrangement.
