Australian Nurse Family Partnership Program Nurse Home Visitor
Winnunga Nimmityjah Aboriginal Health and Community Services LTD (Winnunga AHCS)

The Australian Nurse Family Partnership Program (ANFPP) is a licenced adaptation of the Nurse Family Partnership model developed by Professor David Olds at the University of Colorado, USA. It is a program of sustained and scheduled home visiting for first time mothers that begins during the antenatal period and continues until the child is two years old. The program supports clients to improve their personal health and wellbeing, environmental health, increase their self- efficacy and improve the health and development of their children.

 

The ANFPP involves Nurse Home Visitors and Family Partnership Cultural Support Workers visiting women pregnant with an Aboriginal and/or Torres Strait Islander baby and their families using a therapeutic, partnership approach.

 

The ANFPP National Program Centre (NPC) is responsible for providing extensive education to Program staff from all participating locations engaged by the program and facilitates a national forum for program quality improvement and experiential learning. The Winnunga AHCS Nurse Home Visitor; is responsible for the care and implementation of the Nurse–Family Partnership model within Winnunga’s ANFPP. and early intervention services. This will be part of Winnunga’s broad, holistic clinical services providing culturally appropriate health care for its clients.

We are a community orientated practice and a have an extremely close and friendly working environment. We require a part-time Practice Nurse to work 2 days a week/approx 15hrs. Days and hours are fully flexible.

Some duties include:

  • Spirometry
  • ECGs
  • Immunisations
  • Assisting doctors with minor procedures
  • Management of wounds
  • GP care plans
  • Ear syringe

Experience with Pracsoft software is an asset. Experience in general practice nursing is a must.

We are seeking a qualified nurse to fill the position of Chronic Disease Nurse. The role of the chronic disease nurse is to facilitate and enhance the coordination of care and perform clinical assessments for clients with a chronic condition. Experienced in chronic disease care and the ability to educate clients on chronic diseases. It is envisaged that around 50-70% of the time will be spent developing GP management plans and performing health checks.
