Capital Health Network, ACT’s primary health network, is seeking an experienced Registered Nurse to provide specialised knowledge and support to people with Parkinson’s to facilitate and support self-management of their condition.
Based within the health hub of Deakin ACT, the full-time Parkinson’s Nurse Specialist will have a community focus working with people in their homes and attending appointments with them, where appropriate.
They will also play a key role in supporting the delivery of integrated multidisciplinary care to people with Parkinson’s, including working with GPs, specialists, hospital services, nursing and allied health professionals, aged care and support agencies, and Parkinson’s ACT.
Parkinson’s Nurse Program
The Parkinson’s Nurse Program is a joint initiative between Capital Health Network, Parkinson’s ACT and Rotary Club of Hall. The 3-year pilot program aims to improve the health and wellbeing of people with Parkinson’s.
The program also aims to optimise the available resources for people with Parkinson’s across the health, social services and community sector, to improve care pathways and reduce the potential for hospital admissions and institutional care.
