Apply now: Exciting opportunity for nurses new to Primary Care

Capital Health Network (CHN), ACT’s Primary Health Network in partnership with APNA is excited to announce that we are supporting nurses in the ACT to complete APNA’s Transition to Practice Program (TPP).  The TPP is an 8-month program that aims to build the knowledge, skills and confidence of nurses who have moved into a new primary health care setting, through an evidence-based supportive program.

CHN is providing the program at no cost to successful applicants. We are now accepting applications for places in the October intake of the program. To be eligible nurses must be working in the ACT, and new or transitioning to General Practice nursing.

a) The program

The 8-month program includes the following:

  • Orientation for nurses new to general practice online learning
  • Foundations of General Practice Workbook
  • 10 specific TPP education activities
  • Guidance from a clinical and professional mentor through small group learning
  • Access to self-assessments
  • Nurse support
  • Induction workshop
  • Access to a community of practice.

b) How to apply

Prior to applying applicants should read the following:

  • Program Overview and Requirements – includes details of the program and all the information you require to complete your application.
  • Information for Workplaces – information about the program to provide to your workplace as well as a Statement of Support to be signed by an appropriate representative from your workplace.

Applications can be submitted through APNA’s mentorloop page by 5pm, 2 October 2023 and must include:
