CHN commissions for outcomes by:
- understanding the health needs of the ACT’s communities
 - commissioning services for people at-risk of poor health outcomes and/or exclusion from health services
 - commissioning innovative, evidence-informed primary care services
 - measuring and evaluating for outcomes.
 
We’re proud to have commissioned 47 local service providers to support people at-risk of poor health outcomes, in the areas of mental health, alcohol and other drugs, chronic disease management, aged care and Aboriginal and Torres Strait Islander health.
