Capital Health Network

CHN coordinates and integrates care by: 

  • advocating for and delivering solutions that improve access and experience across the health system, especially for people at-risk of poor health outcomes and/or exclusion from health services in the ACT 
  • partnering to design and deliver models of integrated care that address identified, local needs 
  • delivering programs, projects and solutions that improve communication and coordination of care across the health system 
  • building networks across the health and social sectors in the ACT. 

To deliver new solutions we proudly ran the Social Workers in General Practice pilot and the ACT Breathlessness Intervention Service pilot, both showing positive results for new models of care. In the area of mental health, we’ve partnered with the ACT Health and Community Services Directorate (ACT HCSD) to develop the 2025-2030 ACT Mental Health and Suicide Prevention Regional Plan. To strengthen culturally safe care, we’ve provided education sessions to primary health care professionals, commissioned service providers and CHN staff.
