POLAR Data Tool
POLAR (POpulation Level Analysis and Reporting) is a web-based business intelligence tool for general practice provided by Outcome Health.
POLAR uses data from your practice software to support quality improvement and business development, with the ultimate goal of providing better patient-centred care. General practices in the ACT who share data with CHN can access POLAR at no cost.
MyMedicare
MyMedicare is a voluntary patient registration system which formalises the relationship between patients, their regular practice and GP.
Since the roll out of MyMedicare in October 2023, additional incentives have been linked to the system. Find more information about each incentive by clicking the tile below.
Digital Health
CHN’s Quality Improvement Team can provide support for My Health Record, Electronic Prescriptions, Electronic Referrals, Electronic Requesting, Telehealth and Cyber Security.

Health care Professionals Mental Health Support:
CHN provides access to up to six free and confidential counselling sessions for those working in general practices, pharmacies and allied health settings through AccessEAP. Support is available for any work, family or personal issues without notifying CHN or your employer of service usage.
