Position Description

We are looking for an experienced Registered Nurse to join My Medical Practice Gungahlin to be the driving force and advocate for the local community in preventative health. This role will see you support GPs and their patients to identify and manage their chronic conditions and ultimately keep them out of hospital. Whilst CDM experience is an asset, a friendly “can do” and supportive nature that will fit well with our practice culture is the most important requirement of this role.

In this role, you will be involved in a wide variety of tasks including, but not limited to:

• Prepare Care Plans and Team Care Arrangements to help patients manage their disease/s
• Empower patients to increase ownership over their health
• Coach patients on embedding health behaviour change principles
• Enable patients to access ongoing care from a multi-disciplinary team
• Assist in early identification of chronic disease by conducting health assessments
• Work with GPs to promote preventative healthcare throughout the practice
• Increase the number of patients who receive a care plan for the first time
• Ensure patient’s care plans are reviewed regularly
• Manage administrative tasks relating to carrying out CDM appointment

My Medical Practice Gungahlin is a friendly, patient-centered, family-owned practice located in the relaxed environment of Gungahlin, Canberra. The practice offers flexibility through its multiple locations, including Gungahlin (DPA/AON), Charnwood (DPA), Murrumbateman (MM5), Yass (MM4), and Bungendore (MM5), to cater to diverse patient needs and practitioner preferences. With full nursing support, onsite pathology, allied health staff, and AGPAL accreditation, the practice provides a supportive and professional environment for employees and patients.
