Nonspecific chest pain remains an enduring challenge for GPs. When should patients be referred to the emergency department, who requires urgent assessment, and how should referrals be made? How can we differentiate cardiac from non-cardiac causes? What can GPs do initially to stabilise patients or commence treatment for urgent presentations, and which patients can be safely managed without an emergency department visit?

Breathlessness is a common symptom for life limiting illnesses such as COPD and Heart Failure. It is frightening, distressing and disabling, not only for the person experiencing it, but for family, friends, carers and healthcare professionals.
The good news is, there are things we can do to help manage breathlessness.
The causes of breathlessness are multidimensional and therefore the treatment also needs to be multidimensional.
The Breathing, Thinking, Functional Model (BTF) is a multidimensional evidence-based model to help assess and manage chronic breathlessness. Capital Health Network, together with a multidisciplinary team from Canberra Health Services (CHS), invites you to attend a face-to-face education event on:
This full-day program will deliver practical, up-to-date learning relevant to GPs and primary care providers, covering key aspects of maternity care across the continuum.
Program Topics Include:
- CHS service update and maternity models of care, including the maternity service intake process
- Digital Health Record update
- High-risk pregnancy management and Foetal Medicine Unit (FMU) eligibility
- Obesity, BMI and weight gain in pregnancy, and postnatal weight loss

About Practice Connect
Join us at Practice Connect, Capital Health Network’s quarterly educational and networking event designed for Practice Managers, Practice Nurses and other primary health care professionals to upskill, refresh their knowledge and connect with the primary care community. All registrations include a complimentary meal on arrival and an opportunity to network with like-minded health care professionals.
The Topic
Digital Health is rapidly changing the way general practice delivers care. Over the past year, many new initiatives have been rolled out by the ACT Government and the Australian Digital Health Agency to help general practices manage their workload, increase efficiency and enhance patient outcomes.
Capital Health Network invites ACT Practice Managers, Nurses and other primary health care professionals to an engaging and practical evening, exploring the latest digital health innovations and how they can be embedded into everyday workflows. You’ll learn what’s new, what’s changing and what you can implement now.
The second half of the session will be an interactive, hands-on POLAR training experience. This is a chance to upskill and go beyond the basics of POLAR – from supporting digital health, to improving patient records, predicting vaccine ordering patterns and driving quality improvement. The session will cover concepts in POLAR that aren’t covered in CHN’s foundational training, so you can understand what POLAR has to offer.
We encourage attendees to bring their own device to follow along and apply their learning.

- Understand how DHR Link works and how to organise access for your practice
- Understand Provider Connect Australia and how it simplifies sharing practice updates with partners
- Navigate My Health Record, use it to view MyMedicare registrations and understand updates to Better Faster Access
- Apply POLAR skills for digital health, data quality and practice improvement (bring your own device!)
- Understand what the Walrus decision-making tool is (it’s free!)
- Upload health summaries to My Health Record for the ePIP
- Identify and understand patients using Telehealth
- Record allergies in line with the RACGP 6th Edition requirements

- Explore case examples for QI activities
