Capital Health Network (CHN) is seeking to commission 2 general practices to deliver the Social Workers in General Practice (SWiGP) program in Belconnen, Gungahlin, or other underserved areas in the ACT. The Request for Proposal (RFP) is open to any ACT general practice located in the said areas.

As a first in Australia, Capital Health Network (CHN) funded the Social Workers in General Practice (SWIGP) pilot program which aims to address some of the barriers to accessing health services in the ACT.

The CHN Needs Assessment highlighted social determinants of health as a significant barrier to access and navigate health services, particularly for people with complex social and health needs. There were 52.3% of ACT adults who reported to have a long-term health condition.

SWiGP  was established to increase the level of care coordination in the Territory; build the experience and confidence of GPs to include a Social Worker in the health care team; and improve capacity to support patients with complex needs that cannot be addressed through standard “fee-for-service” consultations e.g. housing, service navigation, psychosocial support, and broader social determinants of health.

The program will aim to achieve the Quadruple Aim and align with the directions set out in the Primary Health Care 10 Year Plan 2022-2031 to:

  • improve people’s experience of care
  • improve the health of populations
  • improve the cost-efficiency of the health system
  • improve the work life of health care providers.

Two general practices will be commissioned to deliver the SWiGP program for a 12-month period. The funding will help the practices establish and embed social work services in their team.

CHN is inviting applications from potential providers in Belconnen, Gungahlin, or other underserved areas in the ACT to submit their responses to this tender.
