About the role
As the Palliative Care Project Officer, you will be instrumental in supporting the implementation of innovative, community-specific initiatives delivered through the Greater Choice for At Home Palliative Care Program (GCfAHPCP). You will work to facilitate improved care and more coordinated end-of-life support in line with the national priorities and the needs of the ACT community. Your role will be key in enabling service integration across the primary and community care settings, bridging care between families, health professionals and service providers.
The successful candidate will focus on delivering a range of targeted activities and collaborative projects that aim to improve awareness, facilitate improvements and build capacity within the primary and community care professionals to support person-cantered palliative care at home.
About the team
The new Project Officer will be part of the Palliative Care Planning Team and report to Palliative Care Planning Manager. The team oversees the implementation of the GCfAHPC Program across the ACT by collecting and generating data to monitor performance, evaluate services and contribute to continuous improvement. The team sits within the broader Health System Integration Team that is responsible for coordinating projects and program areas centred around improvement and integration of the health system to actively respond to the needs of the ACT community.

Work location
This position is based in our newly renovated and modern office, located in the heart of the Belconnen town centre. We support flexibility and a hybrid work model at CHN and are happy to work with the successful candidate to ensure balance between work and life.
