POLAR (POpulation Level Analysis and Reporting) is a web-based business intelligence tool for general practice provided by Outcome Health. It extracts data from your practice software to support quality improvement and business development, with the ultimate goal of providing better patient-centred care. General practices in the ACT who share data with CHN can access POLAR at no cost. 

Benefits of POLAR for your practice 

Polar supports practices to: 

  • Meet Practice Incentives Program Quality Improvement (PIP QI)  requirements. 

– POLAR seamlessly extracts deidentified data from your practice software to submit to CHN for PIP QI requirements.  

– POLAR reports are easy to navigate, allowing your practice to efficiently identify areas of quality improvement against RACGP standards. 

  • Identify patient eligibility for health assessments and care plans. 

– Your practice will be able to provide proactive healthcare, ensuring patients are regularly reviewed/recalled, and that their condition is well managed.  

  • Identify and recall patients for immunisations, screening tests and other preventative health items. 

– You can use POLAR’s system to identify patients to remind and recall, to ensure they are up-to-date with regular preventative health measures. 

  • Use practice data for quality improvement activities. 

– POLAR updates data in 20-minute intervals which is processed once a day. This means that you can view reports no more than a day old without having to manually download an extract.  

– POLAR compiles and displays your data in a comprehensive and easy-to-use graphical format, allowing you to quickly and easily understand your patient population at a glance, and identify quality improvement activities.   

  • Understand and track MBS items.  

– POLAR incorporates and reports on all MBS items, allowing you to identify commonly utilised MBS items, and activities to increase specific MBS item usage. 

– This allows GPs and practice staff to be more informed, with fewer errors in MBS billings and claims, and proper scheduling of care plan assessments and reviews. 

Sharing data through POLAR allows CHN to: 

  • Work more closely with and better understand your practice. 
  • Support your practice with quality improvement activities.   
  • Identify community health care needs. 
  • Provide education in areas of need.  
  • Inform the development of pilot projects. 
  • Provide individual reviews of practice data back to practices. 

If your general practice is participating in the PIP QI and you have signed a new data sharing agreement with CHN you are eligible to have the POLAR tool installed. If you are not participating in the PIP QI and would like to do so, you will need to sign a data sharing agreement prior to installation of the tool.

Process 

Step 1: Your practice will need to sign a new data sharing agreement with CHN. 

Step 2: Once the agreement is signed, the CHN QI Team will create a practice profile for you in POLAR and will contact you to arrange a time for remote installation.  

  • Your practice needs to nominate a POLAR authoriser and administrator. Your authorised person and nominated administrator can be the same person and should be the Practice Manager, Practice Owner or similar (only admin users have access to your practice’s financial information in MBS reports)  
  • The POLAR Administrator can setup and manage your POLAR user accounts for any staff member and allow or restrict access to revenue information in individual reports  

Step 3: Once you have booked an installation time Outcome Health will be in contact to confirm the installation time and instructions on what to prepare for the installation.  

POLAR is only installed onto one server or computer within the general practice or cloud environment. It takes about 30-60 minutes to install including the first full data extraction.
