Role Overview 

As a GP specialising in aged and palliative care, you will be responsible for assessing, diagnosing, and managing the health and wellbeing of elderly patients. Your role will involve working closely with other healthcare professionals, including nurses, physiotherapists, and specialists, to provide holistic and patient-centered care. 

Key Responsibilities 

  • Conduct comprehensive medical assessments of elderly patients with RN support 
  • Develop and implement individualized care plans with pharmacist and RN 
  • Manage chronic conditions and provide preventive care with NP support 
  • Collaborate with multidisciplinary team members 
  • Communicate effectively with patients, families, and caregivers 
  • Ensure continuity of care and follow-up 
  • Maintain accurate and up-to-date patient records 
  • Participate in ongoing professional development and training 

Qualifications 

  • Medical degree and registration with relevant medical board 
  • Vocational Registration with RACGP or ACRRM 
  • No DPA requirement 
  • Excellent communication and interpersonal skills 
  • Commitment to providing high-quality patient care 
  • Ability to work collaboratively within a multidisciplinary team 
  • Experience in aged care is highly desirable
