Information for Primary Health Care Organisations

Capital Health Network (CHN), ACT’s Primary Health Network is offering grant opportunities worth $7,500-$50,000 to primary health care organisations for activities directly related to COVID-19. Total grant funding available is $700,000, through funding from the Department of Health and Aged Care (DoHAC).

The grant recognises the role of primary health care in supporting the prevention, treatment and recovery from COVID-19. It aims to support and address gaps in the health system relating to COVID-19 to improve health outcomes in the community, particularly amongst at-risk populations.

The objective of the grant is to:

  1. Improve access to care and support relating to COVID-19, including post-COVID symptoms.
  2. Reduce the risk of outbreaks through implementing preventative measures.
  3. Upskill primary health care professionals for better management of patients.
  4. Improve staff wellbeing and support staff welfare.

The grant is a one-off funding amount and is not available as ongoing funding, the exact value of the grant will be based on the proposed activity by the applicant.

The grant is available for investments as specified and must be used by 30 June 2025.

The applicant is to identify an activity for funding that directly relates to COVID-19.

This is a ONE-OFF funding amount and is NOT available as ongoing funding.

The grant funding can be used for evidence-based support programs or services for patients, education, resources, staff training, infrastructure, innovation and the reimbursement of staff time associated with undertaking the proposed grant activities.

Where applicable, services and programs must be delivered at no cost to the patient.

Applications focusing on at-risk populations will be favoured.

The funding can only be used for new investments where expenses/costs have been incurred between the activity start date and activity completion date under the Agreement.

Examples of what the grant funding could be used towards:
- An education and support program for people experiencing prolonged symptoms of COVID-19.
- Enrolment to courses and training for primary health care professionals related to COVID-19 and post-COVID symptoms.
- Purchase of video conferencing software to support video telehealth services.
- Purchase of new equipment to support point-of-care services and home visits.
- Reimbursement for staff time incurred for additional hours of training, or developing policies, processes or training materials for practice staff.

The grant cannot be used to cover expenses or costs:
- Business-as-usual costs associated with operating a primary health care practice (e.g. utilities, rent, administration, taxes, consumable products such as personal protective equipment, existing subscriptions or other existing recurrent costs, staff wages/salaries for performing business-as-usual duties).
- For activities or purchases outside of the activity state date and activity completion date under this agreement.
- For the same activities or purchases that the Grantee has received or will receive funding for through another program.
- For major capital works, purchase of land and other retrospective costs or expenses.

To be eligible to receive funding, applicants must meet all the following requirements:
- Be an organisation providing primary health care as described by DoHAC.
- Have an ABN.
- Be registered for the purposes of GST.
- Be located within the boundaries of the ACT region.
- Be performing activities related to COVID-19.
- Be able to demonstrate how you plan to utilise the funding to address COVID-19.
- Nominate a key contact for grant communications.
- Must not have received funding from another source to deliver or provide the same or similar COVID-19 activities.

- No data is required to be shared with CHN.
- The Grantee agrees to provide 6-monthly unaudited Financial Acquittal Reports signed by the Grantee, verifying the Grant was spent in accordance with the Grant Details.
- The Grantee agrees to spend the Grant for the purpose of undertaking the activity only and agrees to meet all the eligibility requirements for the Program as set out in the Agreement.
- Following the activity completion date, the Grantee agrees to provide a final unaudited Financial Acquittal Report along with a statutory declaration signed by the authorised person for the organisation, such as the CEO, CFO or COO.
- Changes in practice details must be reported to CHN.

- Applicants must read and understand the information sheet and agree to the terms and conditions.
- Please submit your Expression of Interest through Microsoft Forms by 5pm, 24 April 2024.
- CHN will review your proposed activity and if successful, will send you the Grant Funding Agreement to be signed by the Grantee.
- Once the Funding Agreement is executed by both parties, the grant amount will be paid to the bank account you have provided and verified via eftsure.

4 April 2024: EOI open 

15 April 2024: Deadline for questions and queries 

24 April 2024: EOI closes

3 May 2024: Funding Agreements provided to successful Grantees for execution

30 June 2025: Grant funds must be exhausted 

31 July 2025: Deadline for Financial Acquittal Report
