Currently, 17,200 Canberrans (4.3% of the ACT population) have been diagnosed with diabetes and it is estimated that an additional 5,000 Canberrans don’t know that they have diabetes.  Current experience suggests that although chronic disease management in general practice is improving in the ACT, there is still room for further improvement.  We know from data sampling that more than 11.14% Canberrans with diabetes had no proper coding of their diabetes on Practices’ clinical information system an (essential step in chronic disease management).

Our team coordinates a quality improvement project with a focus on diabetes. The project involves the implementation of multiple strategies to improve diabetes management and preventative care in general practice. Strategies include improvements in data accuracy and data cleansing, increased diabetes risk assessment and prevention, improved completion of diabetes cycle of care and CDM item numbers together with increased support for patients with diabetes.

Practices need to identify a team member to lead the quality improvement activity and commit to undertake at least four of the above strategies
