Care across the continuum is one of the nine key priority areas for CHN. In the last year, CHN partnered with ACT Health, Southern NSW Local Health District and COORDINARE (Southern Eastern NSW PHN) to provide the ACT & SNSW HealthPathways Program. HealthPathways is a web-based clinical tool that provides health professionals with localised and evidence-based pathways. We also funded Capital Chemist Wanniassa and Charnwood to deliver after hours home-based palliative care medication.
