CHN recognise general practice is faced with increasing challenges relating to system changes, funding constraints, increase in chronic disease management (CDM), practice workforce challenges and COVID-19, just to name a few. GPIT developed a Prospectus to offer support to all facets of general practice within the ACT.

The Prospectus consists of four modules:

  • Process (activities aligned with business improvement)
  • People (activities and support for the practice’s workforce)
  • Patients (activities relating to CDM and Prevention)
  • Programs (information and links to programs and projects delivered by CHN).

The launch of the Prospectus was well received with positive feedback from all recipients. Expressions of interest were received from 42 practices with many selecting to receive support through multiple modules with 580 individual selections made. The top four selected modules included Chronic Disease Management, Diabetes, Income Streams and Cancer Screening. The four program modules, which are key CHN programs, also rated highly. Support for the modules was provided through practice visits, online and telephone support as well as the provision of resources.
